No hospital pays us anything. Everything else about how this company works follows from that one decision.
Your Care Promise is an independent healthcare navigation startup based in India, built inside Speed AI Labs LLP. We help patients in Canada, the United Kingdom and Ireland and Australia, the Gulf, the United States, South Sudan, Zambia, Somalia, Kenya, Uzbekistan, Turkmenistan, Mauritius, the Maldives, Nepal and Bhutan decide whether treatment in India makes sense for them, arrange it well if it does, and coordinate their care for ninety days after they return home.
We’re paid only by patients. We’re not a travel agency, not a hospital marketplace, and not a clinical provider.

Making sure you get independent specialist opinions on your case, and then explaining them to you in plain language. She does not decide anything about your treatment and she does not write the opinion herself. Her job is to make sure the question put to the specialists is the right one, and that you actually understand the answer.

Registered and practising since 2019. Every case that comes to us is read by her before it reaches a specialist, so the question those specialists answer is the right one. Check this registration yourself on the Gujarat Medical Council’s own public search, or on the national Indian Medical Register. Two independent government sources, not one, and neither is run by us. A third, separate confirmation: GMERS Medical College & Hospital, Sola lists a Tutor in Physiology under the same name and the same registration number, on the institution’s own staff page.
Dr Gunjan Patel holds a general medical registration. She is not a specialist in any surgical field, and that is the point: the specialists who review your case are independent of us, and she is the person who makes sure the right question reaches them and that their answer reaches you intact.

Negotiates what you pay the hospital, and guarantees the hospital pays us nothing. Those two jobs sit with one person on purpose. Separate them and the conflict does not go away, it just moves somewhere you cannot see it.
The easier company was there to build. Take the hospital’s commission, let the service feel free to you, and never bring it up. That was turned down, and it is the whole reason anyone here can afford to tell you to stay home. If a hospital, clinic or surgeon ever pays us anything connected to your case, your fee comes back. All of it.
Before the structure
Why this company exists.
When the company arranging your treatment abroad is paid by the hospital it sends you to, the person telling you where to go, and how urgently, has a financial reason to want you to go. This company was built around removing that reason, not by asking you to trust us, but by changing who gets paid for what.
In Nirav’s own words, the aim is to help international patients get treatment abroad in a way that feels like home: choosing the accurate doctor and the accurate hospital for their case, and negotiating on the patient’s behalf the way a family member who understood healthcare would, so the patient’s own family can spend their energy on the patient rather than on logistics.
Nirav Patel negotiates what you pay the hospital, and guarantees the hospital pays us nothing back: no commission, from any hospital, ever. Those two jobs sit with one person on purpose. Splitting them across two people would not remove the conflict of interest that hospital-paid advice carries, it would just make it one step harder for you to see.
Dr Gunjan Patel reads every case herself before it reaches the independent specialist panel that writes the actual opinion. She does not write that opinion and does not decide anyone’s treatment: her job is making sure the right question reaches the right specialist, and that you understand the answer that comes back. We are small, two founders, with no twenty-four hour call centre, and no invented case studies, testimonials, awards or press coverage. We say that openly.
A conflict we're handling openly
Our medical director is also a shareholder.
Dr Gunjan Patel is a doctor and a co-founder of this company. That’s useful, because it means there is a clinician accountable for the quality of what we do. It’s also a conflict of interest if it isn’t handled carefully.
If she wrote your clinical opinion, then the person telling you whether you need surgery would have a financial interest in you buying our case management service. That’s a different conflict from the hospital commission problem we exist to solve, but it is a conflict all the same.
So she doesn’t write it. Independent external specialists do, and they are paid a flat fee that is identical whether they say travel or don’t travel, and they are never the surgeon who would perform the operation.
We could have simply not mentioned her shareholding. Telling you is the same principle as everything else here.

Where we are
Based in India, working across it.
We’re registered in India and we work across the whole of it. The strongest centre for a complex case is rarely the one nearest anybody, so we start from the condition and go wherever the answer takes us.
Being in India matters for one mundane reason. When we ask a hospital for a net price with no commission built in, or ask them to show us the discharge and handover protocol they are already assessed against, we can walk into the international patient department and ask in person.
That sounds unremarkable. It’s the difference between a request that gets answered and an email that doesn’t. A company doing this from another continent, emailing a hospital it has never visited, is doing something much weaker than it appears.
Being straight with you
What we will not claim.
Most companies leave this out. Read it, then judge the rest of the site by the fact that we were willing to write it.
- We have published no case studies, testimonials or patient stories. If you have searched for patient reviews of treatment abroad and found little you could verify independently, that is not unusual: a review needs that patient’s informed consent to publish, and most companies either skip the consent or write the story themselves. We have worked with patients, but none of it is written up here, because we would rather have nothing than publish something nobody agreed to.
- We have no systematic outcome data of our own. We will record it by hand, case by case, from our first case, but we can’t yet tell you how our patients do as a group, because a handful of cases is a story rather than evidence.
- We have no awards, no press coverage and no partnership logos, so you will not find a row of them on our homepage.
- Our legal structure has open questions on it, particularly around advising patients across borders. We’re taking proper legal advice rather than assuming it is fine.
- We’re small. Two founders. If you need a company with a twenty four hour international call centre, that isn’t us and won’t be for some time.
We could have waited until some of this looked better before building a website. But a company whose entire pitch is that it tells patients the truth should probably start by telling the truth about itself.
